New Portal
 
  We are excited to announce that our transition to a new patient portal called Follow My Health is complete. Follow My Health is fully integrated with our Electronic Medical Record and will provide the capability to ask questions, request appointments, view lab/test results, request prescriptions and view your health data. (Online bill pay will be coming soon) Effective 3/23/17 our current portal will no longer be linked to our office, and you will no longer be able to communicate with us through the current portal. You will still have access to the information in your portal account by going to the Medfusion website and logging in. (www.medfusion.net/login) Historical data will also be available through the new portal. Follow My Health will be up and running on Thursday, March 30th. There will be no access to communicate with us electronically through a portal from 3/23/17-3/29/17. We will be sending invitations to set up your account when you come in for a visit.. However, If you already have a follow my health portal with another office, you can go to the Follow My Health website at followmyhealth.com and request a connection to your provider here. We will accept this on our end, and then you will have access. You may also set up proxy accounts for minor family members or family members requiring assistance with their health care (this will require a signed consent) Follow My Health also provides an additional level of patient support if you have issues getting signed in or problems accessing information. There is also a knowledgebase to help guide you through setting up your account, accessing information and communicating with our practice.
